It provides assessments and management to help individuals maintain their health and wellbeing, particularly for those experiencing memory decline, dementia, frailty, falls, or neurological conditions like Parkinson's disease. A team of health professionals, including dietitians, occupational therapists, physiotherapists, social workers, and speech pathologists, offers support both in people's homes and at a clinic located within Royal North Shore Hospital. Programs like the Parkinson's support and strength and balance classes are available to help manage specific health needs. As a public health service, there is no cost for patients who hold a Medicare card.
