St Vincent’s Care

St Vincent's Home Care Brisbane is a not-for-profit home care service helping older Australians remain living independently in their own homes. The service offers clinical nursing, personal care, domestic assistance, social support, transport, home modifications, garden care, and exercise and rehabilitation programs. Funding is available through Support at Home, Commonwealth Home Support Packages (CHSP), or privately. They provide specialist care for dementia and Alzheimer's, hospital-to-home transitions, and chronic illness management including diabetes, heart disease, and COPD, with an emphasis on clinical expertise to improve health outcomes and reduce unnecessary hospitalisation.
