Palliative Aged Care Outcomes Program

The Palliative Aged Care Outcomes Program (PACOP) is a national research and quality-improvement program based at the University of Wollongong, funded by the Australian Government Department of Health and Aged Care. It is not a residential aged care facility; rather, it provides a clinical framework, standardised assessment tools, and education to participating residential aged care providers across Australia. PACOP supports aged care homes to identify and respond to palliative and end-of-life care needs through two components: a Profile Collection for routine resident monitoring and an Outcomes Collection for point-of-care palliative assessments. The program returns data reports to participating homes to support continuous quality improvement in end-of-life care delivery.
